Appointment Scheduling

Appointments are scheduled Monday through Thursday from 7:45am-4:15pm. The office is closed for lunch from 12pm-1pm. We will do our best to accommodate your child’s school schedule; however, as afternoon appointments fill quickly, your child may need to miss part of a school day in order to be seen in a timely manner. Dental appointments are considered an excused absence by California State Law. Our office will be happy to provide you with a signed verification of your child’s visit if necessary. Confirming appointments and cancellation: as a courtesy, our staff will contact you to confirm your child’s appointment the day before. If we cannot contact you, you will receive a voicemail, email and text message in order to make sure you are aware of the appointment. In order to ensure that all of our patients receive the care they need as promptly as possible, our office requires 24 hours notice in the event you need to cancel your appointment. We will always be sensitive to your situation. However, cancellations after 48 hours may result in a cancellation fee.

Payment/Insurance

Before beginning any treatment we will inform you of the costs. If you are unable to pay in full at that time, we will make financial arrangements with you. If you have dental insurance, please fill out your portion of the insurance form and bring it with you to your first appointment. Reimbursement from your insurance company is not guaranteed. Payment for services provided is ultimately your responsibility. Our office offers any of the following payment arrangements:

• Cash or check in full at first appointment (with 5% discount)
• Statement, with 30 day account, payable by the 10th of the following month
• 60 day account, 1/3 due on 1st appointment, 1/3 due in 30 days, 1/3 due in 60 days
• Visa or MasterCard (no 5% discount)
• Insurance (Patient’s portion is due at 1st appointment)
  

In an Emergency

In the event of an after hours or weekend emergency please call our regular office number (408) 265-5880 to be connected with our on call operator. Dr. Rabitz or another local on call Pediatric Dentist will then be paged and will return your phone call as quickly as possible.
